Job Title: Contingent Hospice RNJob Summary:

We are seeking a compassionate and skilled Contingent Hospice RN to join our team at St. Mary's Home Care and Hospice in Athens, GA. As a Contingent Hospice RN, you will provide professional nursing care to our hospice clients, collaborating with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Provide one-to-one care with patients in their homes, focusing on their unique needs and goals.
  • Collaborate with our interdisciplinary team, including physicians, social workers, and chaplains, to develop and implement comprehensive care plans.
  • Assess patients' physical, emotional, and spiritual needs, and develop strategies to address these needs.
  • Provide education and support to patients and their families, empowering them to make informed decisions about their care.
  • Document patient progress and outcomes, ensuring accurate and timely reporting.
Requirements:
  • Graduate of an approved nursing education program.
  • Licensure as a Registered Nurse in the State of Georgia.
  • One (1) year experience as a professional acute care nurse.
  • Home care experience preferred but not required.
