POSITION OVERVIEW:

KEY RESPONSIBILITIES:

  • Act as a liaison among multiple stakeholders.
  • Maintain accurate documentation and records.
  • Conduct assessments to determine eligibility and support requirements.
  • Monitor client satisfaction and the effectiveness of services provided.
  • Facilitate referrals to community resources.
  • Develop tailored service plans for individuals.
  • Provide education and training within the human services sector.
  • Advocate for essential services on behalf of clients.

ADDITIONAL TASKS:

  • Keep up with necessary training sessions.
  • Execute other responsibilities as assigned by management.

REQUIRED QUALIFICATIONS:

  • Strong oral and written communication abilities.
  • Understanding of developmental and intellectual disabilities.
  • Familiarity with treatment methods and intervention strategies.
  • Ability to create and implement individualized service plans.
  • Experience in coordinating with service providers and working collaboratively.
  • Demonstrated respect for clients and their families.

EDUCATION AND EXPERIENCE:

A Bachelor's degree in a human services discipline is mandatory. Experience working with individuals with developmental and intellectual disabilities is preferred. A valid Driver's License is required.
