Job Summary

We are seeking a skilled Nurse Care Coordinator to join our team at Hope. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care and support to individuals with intellectual and developmental disabilities.

Key Responsibilities
  • Medical Record Review: Conduct thorough reviews of medical records and histories to identify potential health risks and develop effective care plans.
  • Healthcare Guidance: Provide education and guidance to individuals and their families on healthcare-related topics, including medication management and disease prevention.
  • Interdisciplinary Collaboration: Participate in team meetings and contribute to the development of individualized care plans that address the unique needs of each client.
  • Communication and Documentation: Maintain accurate and detailed records of client interactions, including progress notes and treatment plans.
  • Professional Development: Stay up-to-date on the latest nursing practices and policies, and participate in ongoing education and training opportunities to enhance your skills and knowledge.
Requirements
  • Education: Graduation from an approved nursing education program resulting in an associate or diploma degree in nursing or a bachelor's degree in nursing.
  • Licensure: Licensure as a Registered Nurse in the State of Illinois.
  • Experience: Successful background clearance, physical and drug/alcohol screening.
  • Skills: Knowledge of professional nursing principles, concepts, and practices; ability to evaluate physical condition and behavior; and ability to instruct non-nursing personnel in healthcare practices.
