Hospice Nurse

Tabitha is seeking a skilled Hospice Nurse to join our team. As a Hospice Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in need of end-of-life care. This role requires a compassionate and empathetic individual who is dedicated to delivering exceptional care and support to patients and their families.
Key Responsibilities:
Conduct comprehensive assessments of patients to identify their physical, emotional, and spiritual needs
Develop and implement individualized care plans that address the unique needs of each patient
Provide ongoing care and support to patients and their families, including pain and symptom management, medication administration, and education on disease management and self-care
Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
Maintain accurate and detailed records of patient care and progress
Requirements:
Current RN licensure in good standing
At least 2 years of experience in acute care or long-term care nursing
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Compassionate and empathetic approach to patient care
