About the Role:
Suncrest Home Health & Hospice is seeking a dedicated Registered Nurse to join our team. As a Nurse Care Specialist, you will provide exceptional care and services to patients in their homes, Skilled Nursing Facilities, and assisted living facilities.
Key Responsibilities:
- Provide psych-social assessments, interventions, and knowledge of community sources
- Work collaboratively with the interdisciplinary team to develop and implement patient care plans
- Manage patient visits, documentation, and meetings with healthcare professionals
- Utilize time management skills to prioritize and triage multiple concerns
Requirements:
- Current Registered Nurse (RN) licensure in good standing
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
- Valid driver's license and reliable transportation
