Nurse Care Specialist

We are seeking a dedicated and compassionate Nurse Care Specialist to join our long-term care facility. This role involves providing high-quality nursing care to residents, many of whom may have psychiatric, dementia, or hospice needs.

The Nurse Care Specialist will float between different floors based on staffing needs and census changes, ensuring continuity of care across all units.

Responsibilities:
Follow physician orders, administer medications, complete procedures, and provide treatments according to resident needs, medical protocols, and nursing standards.

Assess resident conditions and plan, coordinate, and supervise nursing care, including restorative services, ensuring compliance with care standards and facility policies.

Direct and monitor Medication Aides, ensuring appropriate medication administration, watching for side effects, interactions, and contraindications.

Maintain compliance with Civil Service regulations, collective bargaining agreements, and other policies to foster a respectful, cooperative, and safe work environment.

Establish positive working relationships with residents, families, staff, management, and external stakeholders.

Perform direct hands-on care using independent judgment and critical thinking skills to ensure care is in accordance with resident care plans and physician orders.

Develop, document, and implement individualized care plans, and complete sections of the Minimum Data Set (MDS) assessment as needed.

Accurately document care, assessments, observations, and education provided in the medical record, ensuring that progress notes reflect updates to the care plan.

Assist in the orientation, training, and evaluation of Nursing Assistants and other staff members.
Conduct nursing rounds, supervise follow-up care, and consult with physicians regarding treatments, medications, and rehabilitative needs.

Lead change-of-shift reports, ensuring team members are informed of resident needs, and assign tasks based on team members' training and scope of practice.

Ensure compliance with confidentiality and privacy laws, including HIPAA regulations.

Promptly notify the Director of Nursing, Nurse Manager, or House Supervisor of any significant changes in resident conditions and take appropriate follow-up actions.

Respond to medical and psychiatric emergencies, performing interventions as required.

Adhere to safety protocols and promote emergency procedures related to hazardous waste, fire, internal and external disasters, and other safety concerns.

Mentor and support co-workers, new staff, students, and external personnel, encouraging professional growth and learning.
Attend continuing education programs to maintain and enhance nursing skills and licensure.


Requirements:


Education:
Current Registered Nurse (RN) licensure in [State].
