Cardiology Specialist

Job Summary:
Sage Health is seeking a highly skilled Cardiologist to join our team-based care model. As a Cardiology Partner, you will work closely with our care team to provide high-quality care for our patients. Our care team consists of physicians, advanced practice professionals, medical assistants, behavioral health support, quality-based coders, referral coordinators, and more.
Key Responsibilities:
- Evaluate and treat patients in accordance with standards of care.
- Coordinate patient services, including specialty referrals and cardiology subspecialists.
- Participate in the care team and provide medical expertise.
- Collaborate with primary care physicians and nurse practitioners.
- Complete medical record documentation and optimize comprehensive documentation.
- Follow policy and protocol defined by Clinical Leadership.
- Meet with primary care physicians and market medical director to discuss quality of care and medical records issues.
- Focus on direct patient care and administrative tasks related to patient care.
- Support our longitudinal care model for heart failure and disease management.
- Support readmission reduction initiatives for heart failure and CAD.
- Create a framework for upstream stroke prevention.
- Coordinate care for CKD stage 5 patients needing AV graft placement.
- Interpret and document test results to determine heart and cardiovascular system function.
- Coordinate patient care with cardiology subspecialists.
Requirements:
- Graduate of an accredited MD or DO program.
- Current and unrestricted medical license or willingness to obtain a medical license in the state of practice.
- Board Certification in Cardiology.
- Active and unrestricted DEA license.
- Medicare Provider Number.
- Medicaid Provider Number.
- Excellent interpersonal and communication skills.
- Familiarity with team-based care models.
- Ability to learn and adapt to a value-based care setting.
- Superior patient/customer service skills.
- Basic computer skills, including email, EMR, and other software.
Preferred Qualifications:
- Knowledge of HEDIS/STARS.
- Knowledge of CAHPS and HOS.
- Experience working in medically underserved/culturally diverse communities.
- Experience working with an electronic health record.
- Experience working in Medicare Advantage Global Risk Models.
Additional Information:
- No call or weekend work.
- No pediatrics.
Reports To: Network Manager.
