The Role


As we grow our High Risk Programs, we are seeking dynamic and passionate leaders to oversee the people, processes, and technology that set One Medical apart. In the role of Senior Manager for High Risk Care Field Operations, you will report to the Director of High Risk Care Programs and be responsible for fulfilling our commitment to outstanding quality of care and patient-centered experiences.

Your mission will be to deliver high-quality care and service through rigorous performance management and improvement systems. You will lead the operations of a primary care house calls program and related field services across multiple states, focusing on our Senior Health population's highest risk patients.


As a strong leader and innovative problem-solver, you will create efficient workflows and build cohesive teams. Your self-awareness and commitment to continuous learning will guide your focus on the bigger picture. You will excel in customer service, data-driven processes, and motivating your team, especially during times of change.

Key Responsibilities
  • Oversee daily operations of a program that includes virtual care teams supporting in-home primary care house calls.
  • Design and implement standard operating procedures, collaborating with other leaders to determine operational strategies and continuously improve performance.
  • Manage patient identification and enrollment processes to ensure program reach and impact for high-risk populations.
  • Ensure all patients receive appropriate outreach, support, scheduling, and care coordination during their participation in High Risk Care programs.
  • Foster an engaged team culture using frameworks that emphasize effective communication and patient care goals.
  • Communicate and partner with market and in-office care team leaders to ensure visibility of performance and effective patient care handoffs.
  • Travel to various operational locations as needed.
Qualifications
  • 3+ years of leadership experience in programs serving high-risk patient populations, particularly in home-based care models.
  • 5+ years of experience in operations and process management.
  • 3+ years of direct people management experience; virtual team management experience is preferred.
  • In-depth knowledge of value-based care and high-risk services.
  • Proven ability to drive results and support continuous improvement.
  • Experience in talent development through mentorship and coaching.
  • Proficient in data management and analytics tools.
  • Strong written and verbal communication skills.
  • Ability to lead operations from an Eastern time zone.
