The Family Specialist is responsible for fostering a strength-based rapport with children while delivering support, case management, and behavioral guidance. This role emphasizes the importance of the family's voice and choices throughout the treatment process. The Family Specialist intervenes in maladaptive behaviors, aiming to address the child's needs while preserving family integrity. This position requires both independent work and collaboration with a multidisciplinary team.
PROGRAM DESCRIPTION:
The San Bernardino Wrap program is a community-oriented, family-focused initiative designed to support children with emotional challenges in their local environment at the lowest possible level of care. The program's objective is to minimize the risk of out-of-home placements and recidivism by uniting individuals, agencies, and the community as a collaborative decision-making team, with a central focus on fulfilling the needs of the child and family. Services are aimed at addressing the mental health requirements of children and their families, ensuring safe and suitable care, and enhancing coordination and collaboration in care planning with the Children and Family Team (CFT) and other child-serving organizations.
KEY RESPONSIBILITIES:
- Provide a range of supportive and case management services to children in home, school, and community settings, including mentoring, coaching, and behavioral support.
- Identify and reinforce positive behaviors, activities, and skills demonstrated by the youth.
- Maintain sensitivity to family culture, values, and beliefs, operating from a family-centered, strength-based approach.
- Assist youth in developing essential skills for positive interactions with adults and peers across various environments.
- Facilitate transportation for youth to academic, social, recreational, and employment activities.
- Ensure accurate and thorough documentation in compliance with all program, contract, and payer standards.
- Collaborate with the client's multidisciplinary team, including county representatives, schools, and external agencies, to develop and implement treatment plans.
- Acquire knowledge of community resources available to support children and their families.
- Participate in committees, lead training sessions, and represent the program at community events as necessary.
- Perform additional related duties as assigned.
KNOWLEDGE, SKILLS & ABILITIES:
- Proficiency in Microsoft Word, Excel, and Outlook.
- Demonstrated crisis management skills and the ability to apply them effectively.
- Strong organizational skills with the capacity to manage multiple tasks simultaneously.
- Able to establish and maintain appropriate rapport with children, youth, and families.
- Ability to function effectively as part of a multidisciplinary team.
- Understanding of behavioral modification techniques and the child welfare system.
GENERAL REQUIREMENTS:
Education/Experience:
- AA or Bachelor's degree in human services or a related field, along with relevant experience working with youth in mental health or social service settings, or a minimum of 2 years' experience working with youth exhibiting challenging behaviors.
- Bilingual Spanish is strongly preferred.
Licensure, Registration, Certification:
- Valid driver's license and proof of automobile insurance.
- Completion of CPR/First Aid certification within the first thirty (30) days of employment.
PHYSICAL REQUIREMENTS:
- Ability to drive a vehicle without restrictions, often in heavy traffic and for extended distances to ensure safe transport to client settings and appointments.
- Visual, auditory, and olfactory senses sufficient to operate a vehicle safely and identify potential risks in the environment.
- Occasional lifting and carrying of 10-20 pounds.
- Regularly required to sit, stand, climb stairs, and walk during family visits.
- Regularly required to sit and use a computer in office settings.
