Responsibilities- Resident Care: Identify ongoing needs and services of residents, ensuring the highest quality resident services are delivered.
- Assessment and Documentation: Ensure proper follow-through, assessment, and documentation for residents with changes in healthcare services and status.
- Wellness Visits: Conduct monthly wellness visits for all residents, assessing, monitoring, and following up on any falls, accidents, or incidents.
- Communication: Maintain open communication with residents' families and responsible parties to ensure medical needs are being met.
- Medical Support: Provide clinical support and assistance to community team members as needed, ensuring weights and vital signs are obtained monthly for each resident.
- Regulatory Compliance: Maintain wellness files according to Sunrise policies and federal and state regulations, ensuring infection control practices are followed.
- Education: Graduate of an approved college or school of nursing.
- Licensure: Current state license as a professional Registered Nurse (RN).
- Experience: Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
- Skills: Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, as well as federal, state, and local regulations.
