Job Overview

Responsibilities:

  • Conduct regular wellness visits for all residents to identify their ongoing needs and services.
  • Collaborate with the Resident Care Director to evaluate and document resident care services and status.
  • Assist in maintaining resident wellness files according to Sunrise policies and federal and state regulations.
  • Monitor and follow up with residents who have experienced any falls, accidents, or incidents, documenting and reporting as required.
  • Communicate effectively with residents' families and responsible parties to ensure their medical needs are being met.
  • Participate in quality improvement initiatives to enhance resident care and services.

Qualifications:

  • Current RN or LPN licensure required.
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
  • Ability to handle multiple priorities and possess excellent communication and problem-solving skills.
