Job Title: Community Paramedic

We are seeking a highly skilled and motivated Community Paramedic to join our team at AmeriMed. As a Community Paramedic, you will play a critical role in providing high-quality patient care and support to our patients in their homes.

Job Summary:

The Community Paramedic will be responsible for providing in-home assessment and treatment of patients enrolled in our Wellbe Programs. This includes conducting initial in-home assessments, on-going home visits, and participating in discharge planning. The successful candidate will possess strong customer service and communication skills, as well as the ability to work independently and as part of a care planning team.

Key Responsibilities:
  • Provide quality patient care in accordance with clinical operating policies, statewide protocols, and accepted standards of care
  • Attend to all patients in a courteous, professional manner
  • Complete initial in-home assessments and on-going home visits as appropriate for all patients enrolled in the program
  • Participate in discharge planning of Wellbe patients
  • Monitor remote patient tracking and respond as appropriate in a timely manner
  • Complete and submit all documentation/paperwork requirements daily
  • Ensure deployed Wellbe devices are tracked and in good working order at all times
  • Utilize a Home Safety Checklist prior to patient care and report the form to Division Manager
Requirements:
  • Fully state certified as a Paramedic as defined by the State of Pennsylvania
  • Possess a valid driver's license with an acceptable driving record
  • 3 years minimum of experience as a Paramedic
  • The Community Paramedic must be in good Clinical standing pass the scenarios test and PT test
  • Must possess strong customer service and communication skills
  • Must be able to work independently; must be able to function as part of a care planning team
Preferred Qualifications:
  • Advanced Medical Life Support (AMLS), CT, CCP are preferred. State required
  • PALS, BLS, ACLS cards must be current as required by the State of Utah
