Job Summary

We are seeking a highly motivated and experienced Senior Clinical Care Coordinator to join our team. As a key member of our care coordination team, you will play a vital role in ensuring that our clients receive the highest level of care and support.

Key Responsibilities:
  • Conduct comprehensive assessments of clients' needs and develop personalized care plans to meet those needs.
  • Collaborate with healthcare teams, including physicians, nurses, and social workers, to ensure seamless care transitions and coordinate services.
  • Facilitate communication between clients, families, and healthcare providers to ensure that all parties are informed and engaged in the care process.
  • Monitor clients' progress and make adjustments to care plans as needed to ensure optimal outcomes.
Requirements:
  • Current Registered Nurse licensure in the state of Delaware.
  • At least 2 years of experience in case management and discharge planning, with a focus on geriatric care.
  • Proven ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare providers.

Compensation and Benefits

Salary: $85,000 - $110,000 per year, depending on experience.
