VNS Health is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Skilled Nursing professional in Secaucus, New Jersey.
Job Summary:We are looking for a dedicated and experienced nurse to provide high-quality patient care in a home health setting. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently.
Responsibilities:- Conduct initial and ongoing supervisory field visits to patients' homes to provide assessment, training support, and coordination of care.
- Collaborate with the primary care physician and clinical team to perform a full patient assessment and develop a comprehensive care plan.
- Identify continued training needs of the home health aide and document information electronically.
- Deliver the start of care packet and provide an overall review with the patient/caregiver on initial visit.
- Participate in quality assurance and education programs as requested by management.
- Associate's Degree in Nursing from an approved program required.
- Bachelor's Degree in Nursing from an approved program preferred.
- Home care experience in a licensed home care service agency or CHHA settings preferred.
