Job Summary:

We are looking for a dedicated and experienced nurse to provide high-quality patient care in a home health setting. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently.

Responsibilities:
  • Conduct initial and ongoing supervisory field visits to patients' homes to provide assessment, training support, and coordination of care.
  • Collaborate with the primary care physician and clinical team to perform a full patient assessment and develop a comprehensive care plan.
  • Identify continued training needs of the home health aide and document information electronically.
  • Deliver the start of care packet and provide an overall review with the patient/caregiver on initial visit.
  • Participate in quality assurance and education programs as requested by management.
Requirements:
  • Associate's Degree in Nursing from an approved program required.
  • Bachelor's Degree in Nursing from an approved program preferred.
  • Home care experience in a licensed home care service agency or CHHA settings preferred.
