Job Title: Discharge Planner

Frontier Behavioral Health is seeking a skilled Discharge Planner to join our team. As a Discharge Planner, you will play a critical role in ensuring a smooth transition for patients from our inpatient units to their communities.

Key Responsibilities:
  • Coordinate patient discharge planning, connecting clients to various resources such as housing and community services.
  • Provide case management services, including coordination of housing, Medicaid enrollment, insurance enrollment, and coordination of court dates, appointments, or connection with other community resources and follow-up services.
  • Provide 1:1 counseling services to clients as needed.
  • Facilitate psychoeducational groups as a component in the therapeutic treatment plan.
  • Provide outreach and follow-up to individuals discharged from the Evaluation and Treatment Facilities, as appropriate.
  • Coordinate with various mental health and chemical dependency treatment agencies, including DCR's, psychiatric and medical hospitals, and the criminal justice system.
Requirements:
  • Master's degree in the behavioral sciences.
  • Active BLS within 90 days of hire and must maintain active BLS.
  • Must have reliable transportation, insurance, and an active driver's license and be willing to use personal vehicle in the course of work.
Preferred Qualifications:
  • At least one year of related experience working with psychiatrically impaired populations.
