We are seeking a skilled Social Worker MSW or LCSW to join our team at Providence Sacred Heart Medical Center & Children's Hospital in Spokane, WA. The successful candidate will provide biopsychosocial assessment and treatment/interventions/advocacy of patients and their support systems through discharge planning.
Key Responsibilities- Collaborate with patients and support systems to develop and implement evidence-informed person-centered care plans across the care continuum.
- Perform duties in a manner that respects patient self-determination, reduces health disparities/barriers to care, and adheres to the NASW code of ethics and Standards of Practice (SOP).
- Work collaboratively with the Inter/Multi-Disciplinary Team (IDT) to provide comprehensive biopsychosocial assessment and treatment/interventions/advocacy of patients and their support systems.
- Master's Degree in Social Work from a program accredited by the CSWE (Council on Social Work Education) or enrolled in a Master's program to graduate within two months of hire.
- Washington Advanced Social Worker License or Washington Advanced Social Worker Associate License or Washington Clinical Independent Social Worker License or Washington Clinical Independent Social Worker Associate License.
- 2 years of Hospital / Healthcare experience.
- Two (2) years of hospital or healthcare experience.
