Position Overview

Key Responsibilities

  • Design, implement, and assess programs and systems that enhance patient care quality. Exhibit advanced clinical nursing skills in wound, ostomy, and continence care, alongside knowledge of patient care standards and educational consultation.
  • Deliver specialized nursing services for patients experiencing disorders related to the gastrointestinal, genitourinary, and integumentary systems, including wounds, ostomies, tubes, fistulas, and incontinence.
  • Utilize the nursing process to evaluate patient and caregiver needs, ensuring high-quality care for assigned individuals.
  • Formulate and communicate a practical care plan for each patient, actively involving patients and caregivers in the planning process.
  • Execute the medical care plan and nursing interventions to guarantee safe and effective patient care.
  • Assess various elements of the care plan's implementation, including identified issues, care provided, and patient responses, to meet the needs of patients and caregivers and uphold care quality.
  • Maintain accurate and current documentation of nursing care to facilitate effective communication among the healthcare team and ensure continuous quality care.
  • Conduct educational sessions for patients and caregivers to empower informed decision-making regarding treatment options, medication side effects, discharge instructions, and available services.

Qualifications

  • Graduation from an accredited nursing program is required, along with a Master's Degree in Nursing and completion of a WOC Nursing Educational Program.
  • Prior experience in acute care nursing is essential, with a preference for home care experience.
  • Must possess valid Vermont State Registered Nurse licensure upon employment and current certification from the Wound Ostomy Continence Certification Board.
  • Successful completion of background checks, including motor vehicle record, child abuse, criminal record, and adult abuse checks, is required.
