Nurse Specialist

BAYADA Home Health Care is seeking a compassionate and dedicated Registered Nurse (RN) to join our team in the Hospice office, located in Rutland, VT. This office services hospice clients on a per visit basis in the Rutland, Addison, and Bennington County areas of VT.
Responsibilities:- Make home visits to clients in designated geographic territories.
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures, and comfort care.
- Monitor clients' conditions; report changes to Clinical or Client Services Manager.
- Follow up with, execute, and properly document doctors' orders.
- Perform client assessments as necessary.
- Occasional on-call after hours and/or weekends.
- A current license as a Registered Nurse (RN) in Vermont.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience strongly preferred, but not required.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.
