RN Baylor Weekend Care Specialist

Catholic Health Service is seeking a compassionate and skilled RN to provide end-of-life care to patients in their homes. As a Hospice RN, you will be responsible for delivering high-quality nursing care, focusing on symptom management and pain control.
Key Responsibilities:- Provide nursing care to hospice patients, residents, and/or family members in their home setting.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team to ensure comprehensive care and support.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Administer medications and treatments as prescribed by physicians.
- Current and valid NY State license to practice as a Registered Nurse.
- Basic Life Support (BLS) certification.
- One to two years of recent acute care nursing experience with excellent assessment skills.
- Competitive salary range: $95,000 - $125,000 per year.
