Key Responsibilities:

  • Provide nursing care to hospice patients, residents, and/or family members in their home setting, focusing on symptom management and pain control.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct regular assessments and evaluations to ensure the highest quality of care.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.

Requirements:

  • Current New York State Registered Nurse (RN) license.
  • Minimum of one year of recent hands-on experience in home care, hospice, and/or oncology.
  • Basic Life Support Certification.
  • Current, valid NY State driver's license and own transportation.
