Job Summary:

Catholic Health Service is seeking a skilled LPN to provide comprehensive care to patients at home on weekends. The ideal candidate will have a passion for delivering high-quality patient care and a strong commitment to teamwork.

Key Responsibilities:
  • Case manage, assess, plan, develop, and implement a comprehensive home health care plan for patients.
  • Measure vital signs, administer IVs, change dressings, and clean wounds as needed.
  • Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments for various patient populations.
  • Work closely with a supportive patient care team, including physicians, therapy, and social services.
  • Provide patient and family education for disease management and health promotion, directed by physician's plan of treatment.

Requirements:
  • Current LPN licensure.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassion and empathy for patients and families.
