LPN Weekend Care Specialist

Catholic Health Service is seeking a dedicated and compassionate LPN to provide comprehensive care to patients in their homes on weekends.

Job Summary:

  • Case manage, assess, plan, develop, and implement a comprehensive home health care plan for patients.
  • Measure vital signs, administer IVs, change dressings, and clean wounds as needed.
  • Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments for various patient populations in complex home situations.
  • Collaborate with a supportive patient care team, including physicians, therapy, and social services, to ensure high-quality care.
  • Provide patient and family education on disease management and health promotion, as directed by the physician's plan of treatment.

Responsibilities:

  • Deliver high-quality, patient-centered care in a home setting.
  • Work closely with the patient care team to ensure seamless care coordination.
  • Develop and implement individualized care plans to meet patient needs.
  • Monitor patient progress and adjust care plans as needed.

Requirements:

  • Current LPN licensure in the state of New York.
  • Experience in home health care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
