We are seeking a dedicated Case Manager to join our innovative mobile team, serving individuals and families in the community. As a key member of our team, you will work collaboratively with clients to develop personalized treatment plans, addressing various life domains.

Key Responsibilities:
  • Teach and assist clients in improving living, coping, symptom management, communication, and interpersonal skills.
  • Collaborate and coordinate care with other treatment providers, connecting clients with legal, court, and community resources.
  • Provide assistance with crises and help develop crisis plans to ensure client safety.
  • Help clients connect with community healthcare providers and assist with medical and dental appointments.
  • Offer education about mental illness and treatment to clients, family members, and significant members of their support system.
  • Provide support and encouragement in clients' ability to work, attend school, develop friendships, and extend their social network.
  • Advocate for clients' rights and fair treatment in all areas of life.
  • Maintain all required paperwork and charting requirements as established by agency contracts.
  • Achieve and maintain agency standard productivity requirements.
Requirements:
  • Bachelor's degree in Mental Health, Social Work, Psychology, or Human Services preferred.
  • Approximately 1-2 years of experience in the social service field with mental illness or chemical dependency preferred.
