Job Summary:

The Case Manager will work closely with clients, families, and community agencies to provide comprehensive case management services. This includes assessing client needs, developing and implementing individualized plans, and coordinating services to meet those needs. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.

Responsibilities:
  • Conduct comprehensive assessments of client needs and develop individualized plans to address those needs.
  • Provide case management services to clients, including counseling, advocacy, and support.
  • Collaborate with community agencies and providers to ensure seamless service delivery.
  • Monitor and evaluate client progress, making adjustments to plans as needed.
  • Document all client interactions and services provided in a timely and accurate manner.
Requirements:
  • Bachelor's degree in a related field, such as social work, counseling, or psychology.
  • Agency Affiliated Counselor certification in the State of Washington.
  • Active Certified Peer Support Certification in the State of Washington.
  • Valid driver's license and reliable transportation.
  • Ability to pass a TB test and maintain current Basic Life Support (BLS) CPR certification.
