About the Role

The Case Manager will work collaboratively with clients to develop personalized treatment plans, addressing various life domains, including mental health, alcohol/drug issues, and daily living skills. This role requires strong communication and interpersonal skills, as well as the ability to connect clients with community resources and healthcare providers.

Key Responsibilities
  • Develop and implement treatment plans based on client recovery goals
  • Teach and assist clients in improving living, coping, and symptom management skills
  • Collaborate with treatment providers and connect clients with community resources
  • Provide crisis support and develop crisis plans
  • Assist clients in accessing medical and dental appointments
  • Offer education about mental illness and treatment to clients and their support systems
  • Advocate for clients' rights and fair treatment
  • Maintain accurate and timely documentation
Requirements
  • Bachelor's degree in Mental Health, Social Work, Psychology, or Human Services
  • Approximately 1-2 years of experience in the social service field with mental illness or chemical dependency
