About the Role

We are seeking an experienced Patient Care Coordinator to join our team in East Amherst, NY. As a key member of our outpatient clinics, you will play a vital role in ensuring the smooth delivery of patient care services.

Job Summary

\

The Patient Care Coordinator will be responsible for welcoming all visitors to create an exceptional patient experience with every interaction. This includes working in a fast-paced environment, interacting with patients seeking mental health and substance use treatment, and utilizing electronic platforms to manage scheduled appointments and other administrative duties.

Key Responsibilities:

  • Welcome all visitors to create an exceptional patient experience with every interaction.\
  • Work in a demanding, fast-paced environment that is frequently adapting to industry standards.\
  • Interact with patients seeking mental health and substance use treatment.\
  • Utilize electronic platforms to manage scheduled appointments and other administrative duties.\
  • Support the day-to-day needs of our clinical and medical staff.\
  • Creative problem-solving to support the overall operations of the clinic.\
  • Coordinate transportation needs and communicate with referral sources from courts, schools, and hospitals.\
  • Assist with medication refill requests and prepare charts and records as needed.\
  • Manage and triage administrative responsibilities using supplemental technologies to support patient and clinical care.\
  • Develop and maintain accurate and detailed records of patient interactions and outcomes.\
  • Promote a positive and respectful work environment that fosters open communication and teamwork.\

Requirements and Qualifications

To be successful in this role, you will need:

  • A high school diploma or equivalent.\
  • 2+ years of experience in customer service or behavioral health.\
  • Excellent communication and interpersonal skills.\
  • Ability to work in a fast-paced environment and prioritize multiple tasks effectively.\
  • Proficiency in electronic platforms and software applications.\
  • Demonstrated ability to work collaboratively as part of a team.\
  • Familiarity with HIPAA guidelines and regulations.\\
