Job Summary

We are seeking a highly skilled Registered Nurse to join our medical team as a Patient Care Coordinator. This role will involve providing exceptional patient care and advocacy, ensuring the highest level of service delivery.

About the Role

The successful candidate will have a strong background in nursing, with experience in patient assessment, data collection, and goal-directed care planning. They will also possess excellent communication skills, with the ability to work effectively with patients, families, and healthcare teams.

Responsibilities
  • Patient Assessment: Conduct thorough assessments of patients' physical, psychological, and cognitive status to identify areas for improvement.
  • Care Planning: Develop and implement individualized care plans that address patients' unique needs and goals.
  • Communication: Maintain effective communication with patients, families, and healthcare teams to ensure seamless care delivery.
  • Collaboration: Work collaboratively with multidisciplinary teams to provide comprehensive care and achieve optimal patient outcomes.
Requirements
  • Degree from an accredited nursing program.
  • Current licensure as a registered nurse in the state where the facility is located.
  • Minimum of three years' experience as a staff nurse.
  • BLS/ACLS/PALC certification.
Working Conditions

The ideal candidate will be able to lift and/or carry over 20 pounds on a regular basis and push/pull over 25 pounds on a regular basis. They will also be able to stand and/or walk at least 10 hours per day.
