Your Role
Position Overview:
As a vital member of our healthcare team, the incumbent will be responsible for developing, documenting, and executing comprehensive nutrition care plans tailored to the needs of patients in the designated clinic. This role involves conducting thorough assessments, reassessments, and consultations, as well as providing nutrition education to patients and their families. The position also includes participation in quality improvement initiatives and special projects, ensuring that resources are effectively coordinated throughout the health system.
Key Responsibilities:
- Facilitate efficient responses to physician orders and coordinate necessary services, ensuring optimal patient care settings are utilized.
- Exhibit advanced clinical expertise and communication skills, serving as a mentor and role model for staff while ensuring continuity of care.
- Evaluate patients' nutritional requirements and create individualized care plans that reflect their specific health conditions and age.
- Analyze clinical data to develop educational programs for clinical staff, contributing to community health research initiatives.
Qualifications for Success

Required Qualifications:
