Overview of the Role
The Helen Ross McNabb Center is dedicated to enhancing the quality of life for individuals in need. We are currently seeking skilled professionals to fill various positions that align with our mission of improving the lives of those we serve.
Position Summary
As a Case Manager, you will play a crucial role in coordinating comprehensive care for clients. You will manage a specific caseload, collaborating with clients to create tailored treatment plans that address their unique goals and aspirations. Our Case Managers embody the essence of social work, actively contributing to our mission every day.
Key Responsibilities
- Develop and implement individualized treatment plans in partnership with clients.
- Provide ongoing support and advocacy to ensure clients receive necessary services.
- Facilitate access to resources and support systems within the community.
- Monitor client progress and adjust care plans as needed.
- Collaborate with interdisciplinary teams to enhance service delivery.
Additional Roles
In addition to Case Managers, we are also looking for Specialists and Counselors who will provide vital support services, including:
- Information and referral services.
- Agency linkage and advocacy.
- Home visits and follow-up services tailored to client needs.
Qualifications
All candidates must possess an F-endorsement, as transportation may be required for various positions. Salary ranges from $15.93 to $20.05 per hour, depending on the specific role, educational background, and relevant experience.
Work Environment
Positions may involve shift work, including evenings and weekends, particularly for roles in supportive housing settings. Additional compensation may be available for second and third shifts.
