Job Overview

Under the direct supervision of licensed healthcare professionals, the Home Health Aide plays a crucial role in assisting clients with their daily living activities. This includes support with personal hygiene, mobility, prescribed physical activities, meal preparation (including specialized diets), and maintaining a clean and organized living environment.

Key Responsibilities:

  • Provide assistance to clients with personal hygiene tasks such as bathing, grooming, and oral care.
  • Aid clients in moving in and out of bed and assist with walking as needed.
  • Support clients in following prescribed exercise routines as directed by healthcare professionals.
  • Ensure the client’s living space is safe, tidy, and well-organized; change bed linens and prepare meals, including those tailored to specific dietary needs.
  • Monitor and record vital signs such as blood pressure and weight as assigned.
  • Collaborate in the development and execution of individualized care plans in accordance with agency policies; communicate client needs and updates to the healthcare team.
  • Follow all agency policies and procedures, including those related to infection control and safety; maintain confidentiality of client information and ensure compliance with relevant regulations.
  • Participate in regular case discussions to assess client needs and contribute to health education initiatives for clients and their families.
  • Perform additional duties as required by the agency.

Qualifications:

Required Certification: Possession of a valid Home Health Aide Certificate issued by the appropriate state authority.

Additional Requirements:

  • Some positions may require bilingual proficiency based on departmental needs.
  • A valid Cardiopulmonary Resuscitation (CPR) Certificate may also be required.
