Position Overview

Role Summary:

As a Care Coordinator I at Clarvida, LLC, you will play a vital role in providing essential support and intervention for children and families in need. Your responsibilities will include managing client cases and serving as a crucial resource for those seeking assistance. You will actively engage in the case management of children, collaborating with treatment teams to develop and evaluate comprehensive care plans.

Key Responsibilities:

  • Act as an advocate for children and families, connecting them with organizations that enhance their well-being.
  • Facilitate case management processes, ensuring that all aspects of care are addressed.
  • Provide insights during treatment team meetings to support effective decision-making.

Qualifications:

To be considered for this role, you should possess:

  • A Bachelor's degree in a Human Services field from an accredited institution.

Preferred Qualifications:

  • Certification in CPR and First Aid, or willingness to obtain certification prior to client interaction.
  • Experience working with children and adolescents in a therapeutic or community-based setting.
