Job Summary:The Clinical Pharmacist will play a vital role in providing high-quality healthcare services to our patients by assessing patient responses to drug therapy, planning and implementing medication regimens, and collaborating with medical staff to ensure optimal patient care.
Key Responsibilities:- Assess patient responses to drug therapy and plan medication regimens based on physician-established diagnoses.
- Order and evaluate laboratory tests to evaluate drug therapy effects and outcomes.
- Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states.
- Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics.
- Administer prescription or non-prescription drugs according to established agreements or protocols.
- Consult with healthcare providers about patient treatment needs or options.
- Perform a full range of pharmacist procedures, including reviewing patient profiles and ensuring accuracy of pharmaceutical products.
- Provide medication and healthcare-related in-service training to clinical staff.
- Document significant medication interactions and pharmacy interactions with prescribers.
- Conduct Adverse Drug Reaction (ADR) reporting in accordance with The Joint Commission or the Accreditation Association for Ambulatory Health Care, Inc. standards.
- Doctor of Pharmacy (Pharm. D.) degree from a college or university accredited by the Accreditation Council on Pharmaceutical Education.
- Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy.
- Current, full, active, and unrestricted license to practice as a pharmacist.
- Proficient with technology, Electronic Health Records, and Microsoft Office's Suite of products.
- Ability to type minimum 40 WPM and speak, read, and write in English to effectively communicate.
- Current certification in American Heart Association Basic Life Support for Healthcare Providers or American Heart Association Healthcare Provider Course.
The Clinical Pharmacist will work in a fast-paced healthcare environment, collaborating with medical staff to provide high-quality patient care. The ability to lift approximately 10-15 pounds and walk, sit, kneel, bend, and stand for extended periods is required.
