About the Role: We are seeking a highly skilled and compassionate Service Coordination Specialist to join our team. As a key member of our healthcare team, you will be responsible for coordinating and developing personalized care plans for consumers, ensuring their health and well-being are met. This role requires strong communication and organizational skills, as well as a deep understanding of community resources and support systems.
Key Responsibilities:
- Maintain accurate documentation of consumer records, including eligibility for waiver services and individualized service plans.
- Collaborate with consumers, caregivers, physicians, and other providers to develop comprehensive person-centered care plans.
- Conduct intake assessments and annual recertification for new and existing consumers.
- Link consumers to medical services, environmental modifications, and specialized equipment, ensuring seamless transitions and timely interventions.
- Monitor consumer health and welfare through personal visits and phone calls, addressing any concerns or issues promptly.
- Adhere to agency regulations, guidelines, and standards, maintaining confidentiality and professionalism at all times.
Requirements:
- Bachelor's degree in Social Work, Psychology, or related field.
- At least 3 years of experience in resource management or rehabilitation.
- Possession of a valid driver's license and reliable vehicle for home visits and travel.
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
