Join Our Team as a Physical Therapist in Home Health

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team in providing high-quality home health care services to our clients. As a Physical Therapist, you will be responsible for making home visits to clients in designated geographic territories, performing diagnostic tests and measurements, and developing and implementing individualized care plans.

Responsibilities:
  • Conduct home visits to assess clients' physical needs and develop personalized care plans.
  • Perform diagnostic tests and measurements to evaluate clients' physical abilities and limitations.
  • Develop and implement individualized care plans to improve clients' physical function and mobility.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.
  • Provide education and training to clients and their families on rehabilitative care and activities necessary for independent living.
Qualifications:
  • A current MD Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
