Physical Therapist

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team. As a Physical Therapist, you will be responsible for providing high-quality care to our clients in the comfort of their own homes.
Responsibilities:- Conduct home visits to assess clients' physical needs and develop personalized care plans.
- Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Continuously assess and revise care plans to ensure optimal outcomes.
- Collaborate with nursing staff to develop multidisciplinary care plans.
- Provide education and instruction to clients and their families on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations in electronic medical records.
- Current MD Physical Therapist license.
- Minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
