Job Description

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Baltimore, Maryland.

This is a full-time opportunity to provide home health visits to adult and geriatric clients in the Baltimore City area.

As a Physical Therapist with BAYADA, you will be part of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients in their homes.

Responsibilities
  • Make home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements, such as mobility and range of motion assessments.
  • Develop and implement individualized care plans, including manual therapeutic exercises and gait training.
  • Continually assess and revise the care plan, and participate in the multidisciplinary care plan as needed.
  • Educate and instruct clients and their families on rehabilitative care and activities to promote independent living.
  • Accurately document observations, interventions, and evaluations related to client care management and services provided.
Qualifications
  • A current MD Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
