Job Summary:

Responsible for delivering case management, referral, benefits navigation services for clients and providing outreach to the community.

Essential Functions:

Designs, implements and monitors individual program plans for clients. Assesses client needs; investigates their eligibility for services. Counsels and educates individuals and families requiring assistance. Maintains client records and completes required documentation according to agency and accreditation standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs. Coordinates client activities. Assists clients with accessing benefits and services (application assistance, service referral, community partner advocacy, etc.) Networks with community providers to generate community resources for participants and referrals to program.

Presentation to consumer and community partners regarding benefit access and services. May transport consumers to appointments or meet with consumer/partners in the field.

Position Requirements:

High school GED + 5 years human services experience OR Relevant professional credential such as CPRP, CRC, CADC + 2 years related experience, OR A combination of one or more years of college and related work or life experience together equaling four years+ OR Bachelor's degree in human services field, rehabilitation or nursing

Preferred Qualifications:

Spanish Bilingual

Licenses/Certifications Needed:

Valid drivers license in good standing.

Exempt/Non-Exempt Non-Exempt Weekly Work Hours 35 Proposed Salary $40,348/annually ScheduleMonday to Friday 8:00 AM to 4:00 PM
