Key Responsibilities- Assessment and Planning: Conduct comprehensive assessments of patients' physical, functional, psychosocial, and cognitive status to determine their home care needs.
- Develop and Implement Care Plans: Create individualized care plans that establish measurable goals and incorporate therapeutic, preventive, and rehabilitative nursing actions.
- Provide Skilled Nursing Care: Administer medications and treatments as prescribed by physicians, provide health care instructions to patients and families, and counsel patients on meeting their nursing and related needs.
- Communicate Effectively: Prepare clinical notes, update primary physicians as necessary, and communicate with community health-related persons to coordinate care plans.
- Manage Caseload: Act as a Case Manager for assigned patients, coordinating their care and ensuring that arrangements for equipment and other necessary items and services are available.
- Current RN License in California: Must be licensed to practice as a Registered Nurse in the State of California.
- Experience: Minimum 1 year of experience as a professional nurse within the last three years, with experience in public health, home care, or hospice nursing.
- CPR Certification: Must be certified in CPR.
- Driver's License: Must have a valid driver's license and a reliable vehicle.
The hourly wage for this position is $50-$70 per hour.
