We are seeking a highly skilled and compassionate Registered Nurse to join our team at American Home Health Care. As a Home Health Care Professional, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Assessment and Planning: Conduct comprehensive assessments of patients' physical, functional, psychosocial, and cognitive status to determine their home care needs.
  • Develop and Implement Care Plans: Create individualized care plans that establish measurable goals and incorporate therapeutic, preventive, and rehabilitative nursing actions.
  • Provide Skilled Nursing Care: Administer medications and treatments as prescribed by physicians, provide health care instructions to patients and families, and counsel patients on meeting their nursing and related needs.
  • Communicate Effectively: Prepare clinical notes, update primary physicians as necessary, and communicate with community health-related persons to coordinate care plans.
  • Manage Caseload: Act as a Case Manager for assigned patients, coordinating their care and ensuring that arrangements for equipment and other necessary items and services are available.
Requirements
  • Current RN License in California: Must be licensed to practice as a Registered Nurse in the State of California.
  • Experience: Minimum 1 year of experience as a professional nurse within the last three years, with experience in public health, home care, or hospice nursing.
  • CPR Certification: Must be certified in CPR.
  • Driver's License: Must have a valid driver's license and a reliable vehicle.
Compensation

The hourly wage for this position is $50-$70 per hour.
