Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in Farmington, Maine as a Home Health Risk Assessment Specialist on a PRN/Part Time basis.

The successful candidate will conduct adult and geriatric assessments, medical history reviews, diagnoses, treatments, health education, physician referrals, case management referrals, follow-up visits, and clear documentation according to Matrix guidelines and protocols.

Duties and Responsibilities
  • Conduct comprehensive home health risk assessments for adults and geriatric patients.
  • Review medical histories, identify potential health risks, and develop personalized treatment plans.
  • Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary medical personnel as needed.
  • Provide educational resources to patients and families on healthy lifestyle choices and disease prevention.
  • Maintain accurate and up-to-date records of patient interactions, assessments, and treatment plans.
Requirements
  • A Master's Degree in Nursing or a related field, or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant, with strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills, with a positive and outgoing personality, and a commitment to teamwork and adaptability.
Compensation and Benefits

This position offers an estimated annual salary of $65,000-$80,000, depending on experience, plus opportunities for mileage reimbursement and professional growth within the company.
