About the Role

What You'll Do:

  • Develop and implement personalized care plans for patients with complex needs, collaborating with healthcare teams to ensure comprehensive support.
  • Conduct thorough assessments of patients' physical and emotional health, identifying areas for improvement and developing strategies for optimal care.
  • Provide education and guidance to patients and their families, empowering them to take an active role in their healthcare.
  • Utilize evidence-based practices to manage pain and symptoms, ensuring patients experience comfort and relief.
  • Work closely with multidisciplinary teams to coordinate care and address any challenges or concerns that arise.

Requirements:

  • A current R.N. license in the State of Maine.
  • ANA certification in Home Health or Community Health Nursing preferred.
  • A minimum of one year of recent clinical nursing experience, preferably in home health or a related field.
  • A reliable vehicle with insurance for regular travel to patient homes.

Salary Information:
