Job Description

This nursing care coordinator position plays a vital role in providing direct and indirect nursing care to patients assigned to their unit. Working under the supervision of a Nurse Manager or designee, they apply nursing knowledge and skills independently to manage clinical situations.

Key Responsibilities:

  • Patient Assessment: Collects patient data, completes required forms with appropriate responses according to unit standards, identifies patient problems/needs, sets priorities, and refers to multidisciplinary support services when necessary.
  • Care Planning: Develops a plan of care based on the nursing process, incorporates plans from other disciplines, and addresses continuing care needs. Includes the patient/family in developing or revising the plan of care.
  • Care Implementation: Provides treatments/procedures and other care as prescribed and according to patient care standards. Demonstrates understanding of age-related characteristics and needs of patients served, explains nursing procedures, provides discharge teaching, and acts as an advocate and coordinator of patient care with other healthcare personnel.
  • Evaluation: Records patient care delivered as planned and any variation, with appropriate rationale. Makes and records observations related to impending or associated problems and implements nursing measures related to these issues.
  • Leadership: Delegates nursing care to appropriate personnel, integrates cost-effective measures into nursing practice, recognizes unit problems, and takes responsibility for documenting same. Participates actively in Quality Improvement processes, complies with hospital expectations to meet staffing demands based on patient care needs, and monitors unsafe working conditions.

Requirements

An Associate's Degree in Nursing is required, with a Bachelor's Degree in Nursing preferred. One year of experience as a registered nurse is also required. A licensed practical nurse (LPN) experience can be accepted in lieu of one year of RN experience, provided the LPN has completed the Prisma Health Nurse Residency Program or a comparable residency from another organization.

Other Requirements

The successful candidate must possess knowledge and skills necessary to treat patients and other training as required by the department. The ability to work a full-time or part-time status with shifts on every weekend is also necessary.
