JOB OVERVIEW:
POSITION SUMMARY
The role involves delivering comprehensive and/or intensive care coordination for youth and families facing significant challenges. Responsibilities include facilitating in-person, community-based meetings for child and family teams, conducting IMCANS reviews, and providing crisis intervention and safety planning.
Collaboration with various service teams is essential to effectively address the needs of clients referred through Pathways.
KEY RESPONSIBILITIES
- Deliver care coordination and support for High Fidelity Wraparound (Tier 1 clients) or Intensive Care Coordination (Tier 2 clients).
- Engage in consultation and collaboration with clients, their families, and other service providers to execute care plans.
- Conduct assertive outreach, advocacy, and follow-up for assigned clients.
- Maintain thorough documentation of outreach efforts and ensure that all services outlined in the personal growth plan are delivered as scheduled.
- Provide ongoing case management services, including assessment, planning, linking, monitoring, referrals, and advocacy for service recipients.
- Assist clients in developing a natural support network.
- Integrate community resources into the personal growth planning and service delivery processes to enhance various life domains.
- Present relevant information to ensure quality care, gather feedback, and adjust service provision as necessary.
- Identify and report all critical incidents in accordance with organizational policies.
- Facilitate community education initiatives as assigned.
SKILLS AND QUALIFICATIONS
- Strong communication skills across written, verbal, in-person, and virtual platforms.
- Proficient in customer service techniques.
- Familiarity with community resources and their functions.
- Effective problem-solving abilities.
- Understanding of the dynamics related to the assigned disability groups.
EDUCATION
A Bachelor's Degree in a human services field is preferred, or an Associate's Degree in human services combined with three years of experience in mental health services or CRSS certification.
Five years of supervised behavioral health experience may also qualify with approval.
TIME COMMITMENT: Full-time
COMPENSATION: $16.50 - $23.10
