Job Summary:
We are seeking an experienced RN/LPN to serve as an MDS Coordinator in one of our Skilled Nursing Facilities. The successful candidate will possess strong clinical skills, excellent communication abilities, and a passion for providing high-quality care to vulnerable populations.
- Key Responsibilities:
- Develop and implement effective MDS/RAI strategies to improve patient outcomes.
- Lead interdisciplinary teams to enhance care delivery and coordination.
- Maintain accurate and detailed documentation to ensure compliance with regulatory requirements.
- Collaborate with facility leadership to identify opportunities for process improvements.
Requirements:
- CURRENT STATE LICENSE AS REGISTERED NURSE (RN) OR LICENSED PRACTICAL NURSE (LPN)
- Minimum 3 years of experience in MDS Coordination or a related field.
- Knowledge of Medicaid and Medicare regulations.
- Strong analytical and problem-solving skills.
Compensation Package:
The estimated salary range for this position is $75,000 - $95,000 per year, based on experience and qualifications.
