Key Responsibilities:

  • Deliver care in both individual and group settings.
  • Collaborate with primary care providers to offer education and monitoring services for all referred patients.
  • Work closely with staff and providers to identify patients needing education and monitoring services.
  • Independently manage a caseload of patients.
  • Evaluate patients for their educational needs, follow up appropriately, and document in the electronic medical record.
  • Stay updated on advancements in patient care.
  • Provide educational support for nursing staff or others at various locations as needed.
  • Utilize established curricula for patient care.
  • Deliver professional and courteous care to patients.
  • Offer support, education, and ongoing management services to patients.
  • Conduct self-management education in both group and individual settings.
  • Perform other related duties as required or directed.

Required Qualifications:

  • Graduate from an accredited Nursing School.
  • Current RN licensure in the State of New York.

Preferred Qualifications:

  • Experience with Epic software.
  • Background in an outpatient clinic environment.
  • Familiarity with various software applications such as Microsoft Excel, Word, Outlook, etc.
  • Experience with electronic medical records.
  • Demonstrated strong organizational, interpersonal, and communication skills.

New York Residents Only: The hourly pay range for this position is $28.03 to $54.95. Compensation is based on various factors, including education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers a comprehensive benefits package, incentive programs, equity stock purchase options, and 401k contributions (all benefits are subject to eligibility requirements).
