About the Role

We are seeking a skilled Licensed Clinical Social Worker to join our healthcare team in a full-time or per diem capacity. As a member of our team, you will have the opportunity to work with a diverse range of individuals, providing clinical social work services, assessments, therapy, and discharge planning.

Responsibilities
  • Provide Clinical Social Work Services: Work with individuals with diverse needs, providing clinical social work services to support their physical and emotional well-being.
  • Conduct Assessments and Develop Treatment Plans: Collaborate with the interdisciplinary team to conduct assessments and develop treatment plans that meet the unique needs of each individual.
  • Provide Individual and Group Therapy: Offer individual and group therapy sessions as outlined in the treatment plan, providing a safe and supportive environment for individuals to explore their thoughts and feelings.
  • Conduct Suicide and Crisis Risk Assessments: Conduct thorough assessments to identify individuals at risk of suicide or crisis, and develop interventions to support their safety and well-being.
  • Coordinate Discharge Planning: Work with individuals and their families to develop discharge plans that meet their unique needs, ensuring a smooth transition to community-based care.
Qualifications
  • Licenses Required: Current Licensed Clinical Social Worker (L.C.S.W.)
  • Minimum of 12 Months Experience: Possess a minimum of 12 months experience in clinical social work, with a strong understanding of assessment and treatment planning.
  • BLS/CPR Certification: Hold current BLS/CPR certification, demonstrating a commitment to providing high-quality care in emergency situations.
  • Strong Interpersonal Skills: Possess strong interpersonal skills, with the ability to build rapport with individuals from diverse backgrounds.
  • Compassionate and Caring Demeanor: Demonstrate a compassionate and caring demeanor, with a strong commitment to providing empathetic and supportive care.
  • Excellent Written and Verbal Communication Skills: Possess excellent written and verbal communication skills, with the ability to effectively communicate with individuals, families, and healthcare teams.
