Salary Estimate: $65,000 - $85,000 per year, depending on location and experience.

Job Description:
We are seeking a highly skilled and compassionate Home Health Social Worker to join our elite team of healthcare professionals. As a Medical Social Worker with Elara Caring, you will have the opportunity to make a meaningful impact in the lives of our patients, providing patient care that exceeds expectations.

Your Key Responsibilities:
  • Provide high-quality patient care across the entire continuum
  • Collaborate with interdisciplinary teams to develop and implement effective care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and implement individualized care plans to address patient goals and outcomes
  • Provide education and support to patients and their families

Requirements:
To be successful in this role, you will need:
  • A Master's degree or Doctoral degree in social work from a school of social work accredited by the Council of Social Work Education
  • Current state license as a Social Worker
  • At least one year of social work experience in a healthcare setting
  • Excellent communication and interpersonal skills
  • A valid driver's license and reliable transportation
