Renal Care Specialist

We are seeking a skilled Master Social Worker to join our team at Innovative Renal Care. As a per diem social worker, you will play a vital role in supporting patients with End Stage Renal Disease (ESRD) and their families.
Responsibilities:- Assess patient needs and develop personalized care plans to ensure optimal health outcomes.
- Collaborate with healthcare teams to provide education and support to patients and their families.
- Conduct home visits and provide emotional support to patients in their homes.
- Develop and implement community outreach programs to promote health education and awareness.
- Participate in quality improvement initiatives to enhance patient care and services.
- Master's degree in Social Work (MSW) from an accredited institution.
- Current state licensure (e.g. LCSW, LMSW) and meet all state regulations to practice in a dialysis setting.
- Two years of clinical experience, preferably in an ESRD setting.
- Current CPR certification preferred.
- Excellent communication and interpersonal skills.

- Annual raises significantly above the industry standard.
