Renal Care Specialist

We are seeking a skilled Master Social Worker to join our team at Innovative Renal Care. As a Master Social Worker, you will play a vital role in helping patients understand their rights and responsibilities, and guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
Responsibilities:- Assess patients' needs and develop personalized care plans to ensure optimal outcomes.
- Collaborate with patients, families, and healthcare teams to provide education and support.
- Conduct regular assessments to identify and address any changes in patients' conditions.
- Develop and implement strategies to improve patient engagement and adherence to treatment plans.
- Provide emotional support and counseling to patients and their families.
- Master's degree from an accredited school of social work (MSW).
- Current state licensure (e.g. LCSW, LMSW) and meet all state required regulations to practice in a dialysis setting.
- Two years of clinical work experience, preferably in an ESRD setting.
- Current CPR certification preferred.
- Excellent communication and interpersonal skills.

- Annual raises significantly above the industry standard.
- We recognize your most recent dialysis experience to calculate paid time off accrual.
