Renal Care Specialist

At Innovative Renal Care, we are seeking a skilled Master Social Worker to join our team as a Dialysis Social Worker. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals with End Stage Renal Disease (ESRD).
Responsibilities:- Assess patients' physical, emotional, and financial needs, and develop personalized care plans to address their unique challenges.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and optimal patient outcomes.
- Provide education and support to patients and their families on ESRD management, treatment options, and resources available to them.
- Conduct thorough assessments of patients' social and emotional needs, and develop strategies to address these needs and promote overall well-being.
- Work closely with the interdisciplinary team to ensure comprehensive care and address any concerns or issues that may arise.
- Master's degree in Social Work (MSW) from an accredited institution.
- Current state licensure (e.g., LCSW, LMSW) and meet all state regulations to practice in a dialysis setting.
- At least two years of clinical experience in a healthcare setting, preferably in an ESRD environment.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
