Wound Care Clinical Coordinator

At MedAmerica, Inc., we are seeking a highly skilled Wound Care Clinical Coordinator to join our team. As a Wound Care Clinical Coordinator, you will play a vital role in our mission to improve lives by providing exceptional patient care and coordinating clinical activities of the Wound Care Center.
Key Responsibilities:- Provide case management to a group of patients, conducting specific assessments according to protocols and procedures, and delivering clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management.
- Organize and prioritize clinical responsibilities of clinical staff, including medical assistants and wound care technicians.
- Collaborate with other healthcare providers, key hospital personnel, Wound Care Center Providers, and Medical Director regarding clinical and patient needs.
- Consistently look at current processes and procedures, and identify and implement areas of improvement, while complying with Health Insurance Portability and Accountability Act (HIPAA) and the Department of Health (DOH).
- Develop and maintain a system of "care consideration" of our primary care patients to ensure the front office schedules the patient to be seen for appropriate tests/therapies.
- Engage in staff development, providing general in-services as well as individual instruction and orientation.
- Provide staff recognition through positive reinforcement and constructive feedback.
- Ensure that the clinic adheres to all relevant healthcare regulations, standards, and guidelines.
- Implement quality assurance measures to monitor and improve the quality of patient care.
- Conduct regular audits and assessments to identify areas for improvement and address any compliance issues.
- Collaborate with Medical Director to achieve Wound Care Center quality indicators.
- Perform patient care based on Center staffing needs.
- Collaborate with Medical Office Coordinator and Medical Director with all-staff meeting and actively participate in ongoing provider meetings.
- Ensure daily operations and steady progress towards established goals.
- Establish and maintain open and effective communication with all members of the staff.
- Current active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice is required.
- Two (2) years of direct patient care experience required.
- Current Basic Life Support (BLS) certification is required.
- Clinical nursing experience with a focus on wound care preferred.
- Experience working in an acute care or rehabilitation setting preferred.
- Proficient in all responsibilities related to Case Management.
- Strong clinical assessment and critical thinking skills.
- Effective communication and interpersonal skills.
- Ability to develop and maintain effective relationships with patients, families, staff, providers, and all other members of the health team.
- Ability to work collaboratively with our team.
- Proficient with advanced wound care technologies and techniques.
- Solid understanding of medical insurance plans and office procedures.
- In-depth knowledge of medical documentation.
- Strong clinical assessment and critical thinking skills.
- Ability to demonstrate leadership skills to delegate and provide direction/guidance to others on team.
- Able to stay organized and flexible in a work environment with multiple assignments and changing priorities.
- Ability to uphold Health Insurance Portability and Accountability Act (HIPAA) requirements.
- Highly organized and able to manage multiple priorities.
- Ability to identify strategies to improve patient satisfaction and outcomes.
